• I understand that this service is NOT a medical service and is NOT a crisis service.

• I understand that counselling does NOT involve the provision of direct advice and I accept full responsibility for my decisions and actions.

• I understand that the information I share during my session is confidential and will not be disclosed to anyone without my consent, except if:

– There is a legal requirement to provide information to a court of law; or

– There is a serious safety concern.

• I will contact my counsellor if I do NOT wish them to take any notes of my session.  Otherwise, my counsellor may take notes of my session and will store them in a digital, secure format.

• I understand that full payment will be required prior to securing my booking.  If I wish to cancel my appointment, I must provide at least 24-hours of notice to receive a full refund.  Otherwise, no refunds for cancellations or for non-attendances will be given.
